Chronic Disease Care
Ongoing support to help manage long-term health conditions and improve quality of life.
Chronic disease care at our medical centres focus on supporting patients living with long-term conditions such as diabetes, asthma, heart disease, and other ongoing health concerns. The GPs and nurses work closely with patients to develop personalised chronic disease management plans that aim to improve health outcomes, reduce complications, and support day-to-day wellbeing. Care plans are tailored to each individual and regularly reviewed to ensure they remain effective and up to date.
Our nursing team plays an important role in chronic disease care, working alongside your GP to provide education, monitoring, and practical support. This may include health assessments, care plan reviews, and coordination with allied health professionals such as dietitians, physiotherapists, and specialists when required. Through a collaborative and patient-centred approach, we aim to empower patients to better manage their condition and maintain an active, healthy lifestyle.
